fiunally, it only shows minimum 3.... if there's less than 3 it will be blank... so, even if the icon is not orange, there may be one or two things to review.
a number of bugs about this have been filed on various meta's ;-)

If you have almost 32K answers on the site..... all you need is 0.032% of them to be upvoted each day....
On June 28, he answered 1 question. It got 2 votes. He got 200 rep, and then another 40 or more votes that were capped.
 
Does he get all those votes because he is so well known across SO and other SE sites?
 
Not really.
People don't just serial upvote him.
It is a lot of people just coming and his answer solves their problem - it's happened to me a few times.
Plus, he still is active, so I'm sure that helps somewhat.
