Well that makes me feel better. Maybe I'll be less worried next time :P
3 of those weren't "our" fault: they had heart attacks on the course, and those happen fast enough that nobody could get to them with an ED in time.
2 of them still weren't our fault, but were directly related to the use of the course: customers removed their safety lines from the system and either fell or jumped.

There've been a bunch of major injuries (broken bones, etc), but honestly... all the incidents we have are a direct result of people either being dumb or not following safety procedures. Listen to what instructors tell you, and the thing is as safe as anything else in life.

I'd say misunderstanding directions is still at least partially the fault of the course.

At the same time... we've had 7 million customers, and only one or possibly two of them managed to misunderstand directions to that extent.
It's probably less likely than getting crushed by a vending machine.
There's a couple hundred incidents logged for my site, but 90% of those are "customer recieved minor cut to {insert body part} due to {insert cause of injury}, which was in direct contradiction of the safety rules that were explained to them at the start of their session"

I think the worst injury we've had at my site in the 4 months it's been open is a sprain or two.

Really, though, it's easy to fake an interview. Especially for an intern where you're trying to gauge willingness to learn, not actual technical ability... almost impossible to hire accurately.

@Henders So we make everyone sign a disclaimer before they get anywhere near a harness. The script for doing that goes along the lines of "read through these points at the top, then we need name, age, and supervising signature for everyone participating". I'm still amazed at the number of people who, despite just being told to, refuse to read through the 7 short sentences at the top of the page that describe exactly what they're signing away.
It's a bit like 'Well I have to sign this to do the activity therefore I will just sign' for some people, I guess.
