weird, do you use privacy badger ? if yes allow cookies from metasmoke manually and then remove FIRE cookies and database with the console in firefox and restart from new giving new access for fire on metasmoke and as well allow fire to act on your behalf on SE network
@Makyen I have autoflagging enabled, but MS has only used one autoflag. Could of course be random, but with the amount of time since I set it up it's too random

@ArtOfCode PB and Disconnect. PB is set to whitelist charcoal-se and metasmoke. Disconnect doesn't block either. Disabling both doesn't make a difference
@MartinSleziak I tend to leave a comment on those because a few moderators (networkwide) might not understand why we flag that as spam, especially if there's no answer yet.
FYI @Makyen I've just reset the max flags to 3, so if autoflagging gets turned on for some reason without anyone fixing the problem, we have at least some sort of safeguard
(I haven't seen the implementation for the 4th flag yet, it's possible that it won't make a difference at all, given that it's broken now)
The failure in placing the System flag is certainly the thing that stands out with that post. The most likely thing is it doesn't properly recover, in at least one of several possible ways.

@tripleee suspiciously, that's the same time autoflagging was disabled. Not that I know how that would affect it. How does it usually get the notifications?
There are sub-types of that, though. What I'm thinking is that if you're listening for flag_log or not_flagged... neither of those are going to get sent, because autoflagging's disabled.
Recommendation (requires some work, but gives you a much better guarantee): switch to WS API 2.0, listen for posts#create
