what would convention say is the answer to those 2 questions?
 
"Test" would be the "first element" in the array.
But it would be at "index 0."

I would say that it is always the index

So, if you had an array: "Hello", "World", "Hello" would be the first element, at index 0, position 1, and "World" would be the second element, at index 1, position 2.
