That's a tricky problem to solve. Obviously, you can't disincentivize providing emergency care, because that will result in people not getting the emergency care they need. But people/insurance can only afford to pay so much (more) for preventative care
 
Long term, preventative care is cheaper.
In one state, the department of medicaid gave the large hospital network some fixed amount of money and a stipulation that they were not allowed to turn patients away
Previously, this hospital network had been closing primary care facilities.
So, given this fixed amount of money, they reopened them, and made a great profit
The system in its current state incentivises building more and more hospital beds rather than investing in primary care, because a hospital bed will make way more money than a primary care facility ever will
