Is SQL Server based on Relational model or navigational model? or is it RDBMS or DBMS?

I thought it was both.
 
Logically, it is largely relational. Physically, quite a bit is navigational.
But it is as 'R' as pretty much any other mainstream SQL product.

@cpx Any DBMS can be used to implement a non-relational data structure. The defining characteristics of a RDBMS are to do with its support for relational operations such as joins.

Same as the OP in the question I only see it with four cross joined sys.objects. Doesn't show up with three.

@MikaelEriksson Maybe because for less than 4 there is always a rowcount spool on the inside of the NL rather than direct scan.
