There's like three different possible cases for failing without creating a flag log, none of which would create logs so
but since it works on some posts and not others I'm tempted to point the finger away from "no one has API tokens"

Current best theory: we made an API boo-boo, and no one was able to get flag options at all
Check for long-running threads and/or maybe try and cast a manual flag
we need better logging for this

@Undo confirmed that HL did not scan it, which means send_not_autoflagged never ran.
you should run htop to see if it's still running, I can't envision any scenario where it could have not done that without raising an exception

I would say at least a good third of posts are failing completely somewhere in here (no flagged or not_flagged messages broadcast)
You can tell by looking at Half-Life's output: see how it's skipping IDs: chat.stackexchange.com/rooms/65945/charcoal-test
You wouldn't see it in /flagging/logs because there were no flag log fails
Undo left so I can't tell whether the autoflagging threads are zombie'd
 
we spawn new threads for each post, though, so that wouldn't cause more than one failure

I mean if they're all still alive and stalled
Because post.send_not_autoflagged is not getting called
and I can't think of a branch where that would occur without an exception being raised, which triggers an automatic fail

the HL output is a decent way of seeing it, if the ActionCable logs are bork

SO seems to hate suggested edits
I've never seen a formatting edit, no matter how substantial, that doesn't get at least one reject
 
Anywhere as big as SO will have its share of bizarre reviewers
 
it's not like these people have an unusually high number of rejects
and I'm not kidding when I say "every"

Mods and OPs being able to retroactively reject/approve edits helps a lot
