It reports posts that match various regexes (or more complex processing too, I think) and blacklisted users (generally due to past spam). Yes, it's site-for-room tunable.
 
And rules per site tunable.
 
Smokedetector currently reports to the following rooms: Chat rooms
 
If you would like it to report spam for a specific site in a specific room, feel free to drop a message here, we will add the rules then.

Indeed, as are the considerate users in this (and possibly other) rooms. I'd wager it would be hard to find enough site regulars to even flag-delete a spam post in 2 hours, let alone minutes. And apparently this year we're suddenly starting to get the good stuff on a daily basis.
 
Reports of posts that live longer than five minutes get escalated to Tavern on the Meta for more eyeballs, so that would be the other room. (SO posts go to the SOCVR room. I don't know what other rooms are active.)

@ByteCommander Unfortunately, an edit within grace period does not generate a new "activity" for Smokey to detect. There is nothing we can do when spammers use this tactic, short of maybe checking some already-checked posts again when fetching via the API.
@JeffreyBosboom Titles of new questions are scanned immediately, since they are provided by a websocket. The queue delay applies only to body of posts.

@jokerdino If you really want it... but destroying users doesn't add much of a SpamRam penalty (just spam-flagging the post into deletion should be enough).

@jokerdino Nothing wrong, for sure. But it's a question of where to put the limited amount of effort. Money.SE mods used to destroy every account identified by Smokey (who reports in their chatroom). This didn't make a difference for how much spam the site got, so they eventually got tired of clicking those extra buttons.
 
Remember that not all detections from smokey are always spam, we keep statistics what reasons detect spam with what accuracy they have
