So, I was playing around with a much more verbose version of my Twisting Ropes answer and stumbled across a situation I've never noticed before:

I parented an empty to the curve using Ctrl-P instead of the parent field in the object properties. The position of the object in the N panel appeared to be in world coordinates instead of local coordinates. Poking around with python I discovered the "secret" matrix_basis property was not the identity matrix.

In cycles the image being painted is determined by the active image texture node in the material nodes
