It's not necessarily talking about a blood relation. (In fact, are the Starks related to the Targaryens if you go far enough back?)

Also, the conditional helps. Without context, the title could mean "What would be the relation between them if R+L=J is true?"

@Olórin this isn't future works any more. Future works is a particularly tricky close reason, because the closure might have been perfectly valid when those votes were cast and yet the question should absolutely be reopened now. There's almost always a clear 'sell-by date' for FW closures.
