@DavidCarlisle I just updated everything on my Linux, using steps shown here askubuntu.com/questions/163682/… and still get the same error. looking at versions of tex4ht and latex after running the above steps in the web page, nothing changes in Latex on my Linux box.
@DavidCarlisle, Ok, but how? can I download the .sty file manually from CTAN? Where did you say I should put it? Should I overwrite these from whatever is in CTAN now?
@Nasser well as a test you can just dump al the new hyperref files in the same directory as your document then tex will use them. or TL2013 is nearly ready as you say:-)
@DavidCarlisle, Ok, I still get the error. I downloaded http://tug.org/applications/hyperref/ and put all the files in the same folder:

@MarcoDaniel No, but it does contain full indexes of a lot of the major journals in the field. It actually was created by a friend of mine who I think had some money to pay a student to enter all the data (not by hand, of course.)

With document structures being so different, I'm tempted to do a personal investigation into eliminating these problems of "duplicate destination names" and perhaps provide a canonical question/answer on that...
@PauloCereda Ah, that could be. When I googled trap 6 I got stuff that was about errors in a variety of languages. It also might be related to Mountain Lion.
