@MarcoDaniel: Is there a easy way to get access to the corners of an mdframed environment? I am using the usual \tikzmark, could simplify my code if there was an easier way. I want to do a drawing just at \end{mdframed}. If you think this warrants a proper question, I can post it.

@PeterGrill Souped-up \tikzmark now available. As I say in the answer, in functionality then it's not too different to Stephan's answer in terms of what you want, but it's a cleaner implementation of the idea behind \tikzmark and has some definite advantages over the original (such as being able to use before the mark is declared and thus put drawings under text).
