@JackDouglas SQL Server does not implement any facility to seek directly to the next distinct value in an index skipping duplicates along the way. I thought that was what the skip scan did in Oracle. Guess I was wrong about that then? For SQL Server if there are lots of duplicates then a recursive CTE can be faster (repeated index seeks) as @Sqlkiwi illustrates on this post
