Seriously, though, here are some ideas I had, none of which have panned out for me yet but maybe one (or more) of them will help someone:
Heart attack = MI, or anagram of HEART
stricken leader = anagram of LEADER, or means that the preceding thing is removed from a synonym of 'leader' (or a name of a leader)
turning pastier = some synonym of 'turning pastier' or an anagram of PASTIER or (IMO less likely) turning= an anagram of LEADER and pastier = a synonym of 'pastier'
pastier struggling can also = anagram of PASTIER
lots of anagram signals here <angry face at @GarethMcCaughan > :-)
I also had the following idea, but I highly doubt most cluesetters would do it: 'pastier' (or maybe 'turning pastier') is a signal for more-glued-together, so for example it could be LEADER turning pastier --> L+E, A+D, E+R --> Q, E, W
I really like the idea of MIS + <synonym of 'turning pastier'> = <synonym of 'struggling to breathe'> but can't seem to find appropriate words for that.
(Oh, I didn't mention, that's from stricken leader = S.)
Oh and of course struggling to breathe can be an anagram of TOBREATHE
Dunno if any of that helps anyone. Anyone want to share any other ideas?
Oh and of course the definition half could be 'turning pastier, struggling to breathe', but that seems unlikely to me

Oh and one more I thought of earlier but forgot to mention here (and there are probably more, too, but for now I've remembered one): Heart attack stricken = INFARCTED, and the rest is the wordplay half somehow.
