Howdy strangers, would like to know if I am starting to go overboard with references, (integrity?, normalization?).
First off, I have this example design (https://i.gyazo.com/8da768ad117c1b6467c4a37edefa5171.png), do tell me if anything jumps out as silly.
The idea with groups for example, is I can JOIN entities, type and ?groups to get all the information about a particular entity. Is this silly? Should everything should just work off entities, for example group_members, even if there are no other columns in the group table other than the primary key? My reasoning behind designing it this way, is to separate some concerns, for example, not every entity is a group.

@avitex If groups is an abstraction for teams and orgs, perhaps the latter two should be referencing the former. Other than that, everything makes sense to me so far. I'd probably take further steps to make sure a users.entity_id cannot reference an entities.id whose type is O or T.

@yaapelsinko could you answer Aaron's question about where you check your memory usage? I don't think the question is overly clear at the moment. Also what are the min/max memory settings on your SQL Server instance, and if you say there are other applications on the server, which ones? What is their memory usage? You say SQL uses 200MB memory, so why do you think it's SQL taking up all the RAM? Please clarify your question. — Tom V 16 secs ago

@Lamak - please read the very first comment under Gordon's answer. You will see that I asked the OP for clarification. It is entirely sensible that, without further clarification from the OP, one might assume that the values of CC_CODE were restricted to the same small number of values. They aren't, which - with that knowledge - makes the answer incorrect. In the same comment, before the "new answer" was ever posted, I also suggested how Gordon's answer can be easily adapted to the more general case. Do you see it now?
