To be clear what I would say there is that the name of the permanent that was put on top of the library would still be free information, along with the fact that it was put on top of the library, and the fact that the library was later shuffled.
In the situation in the original question, there is no practical difference between knowing the name of the card at the time it was put face down on the battlefield, and knowing the name of that same card that is currently face down on the battlefield.
Similarly with the library thing, before the library is shuffled, there is no practical difference between knowing the name of the card at the time that it was put on top of the library, and knowing the name of the top card of the library.

my point is, I don't think the name of the card is included in the definition "Details of current game actions and past game actions that still affect the game state.", based on the name of a card having nothing at all to do with any game action, and of course it not being stated anywhere in the rules. The reason I concern myself with the specifics is because it makes all the difference whether what's in my answer having any merit
all I see the rules state is "The name of any visible object.", which is rather specific, and would probably be phrased "The name of any visible or previously visible object." if it intended what you're suggesting is the case. There is good reason why names of non-visible objects aren't free information in my opinion, for example because players shouldn't be required to remember every single card's name they put on top of a library over the course of the game, say in a limited sealed match
yet they would be, because they're supposed to provide free information to their opponent if asked. now I realize this is not a case where any judge would come down on the asked player, but nevertheless the rules are there to provide a solid ground work on how to handle situations, and if you say hidden information can sometimes be free information, that turns from solid into a bit of a mess, in my opinion

@TheThirdMan I don't understand how details of an action could possibly not include information that was available at the time about the objects that were acted upon
And those are two separate bullet points in the free information list. The fact that one bullet point lists "The name of any visible object" does not mean that none of the other bullet points can also make the names of other objects free
