@CowperKettle I think it’s fine as “What exactly are we doing wrong” The other word order seems off to me with “exactly” - I would write “Exactly what we are doing wrong.” If I moved the “are” and I’ve highlighted the difference in emphasis if I were to read them aloud as a title.

@CowperKettle In the second version I might put the emphasis on “exactly “ the more I look at it, the more unsure I am that my first instinct was correct. But then I think about how I’m usually wrong when I second guess myself. Now I’m wondering if I’m third-guessing... so it’s safe to assume you might hear a slight emphasis on “exactly” or on “what” and neither is strange...
