Basically, yes. The underlying reason is that if you chain a single [in]equality operator like a < b < c < d or a == b == c == d it gets parsed as multiple arguments for that operator (Less[a,b,c,d] or Equal[a,b,c,d]). Once you start mixing them, you'll get an Inequality that contains both the operands and the operators (and which does pairwise checks and ANDs them all).

So the second example gets parsed as Inequality[1, Unequal, 2, Unequal, 1 Equal, 1]. Normally this is completely indistinguishable. If you write Inequality[1, Less, 2, Less, 3] that's the same as Less[1, 2, 3]. But Unequal is the one exception where passing multiple arguments to Unequal doesn't just check consecutive pairs, but all pairs instead.
