Is there a MMA supported format that can be used to import .env / dotenv files with:
KEY=VALUE
...
structure?

you could try a quick hack by importing the .env as a string, replacing any '#' comment characters with a ';', and doing an ImportString[str,"Ini"]:

envstr = Import["~/scratch/test.env", "String"];
envstrr = StringReplace[envstr, "#" -> ";"];
envimp = ImportString[envstrr, "Ini"];
