The existence of these companies is a natural side effect of the UK healthcare system for trans people being underfunded and overloaded; trans people going through the public system typically have 4+ year wait times before they can even begin appointments to get medication. Most people go through private if they can.
The news article didn't examine the root cause though — the fact our system is broken.
The worst part is: trans people depending on this medication may actually die, if they've had any kind of GCS. If your body doesn't have hormones to use... that's lethal.
(transphobia kills, y'all)

@doppelgreener Are you talking about exogenous glucocorticoids?

All 160 lbs of me at the time lol
ah, high school. 165 lbs. 35lbs ago.
@NautArch ah, high school. About 5’3”. Still.
@BardicWizard By the time I got up to 165, i think i was about 6'
16:23
@GcL Gender confirmation surgery. If you've had that, you probably no longer have gonads, and there's some hormones your body no longer produces—you're instead using HRT as your hormone supply.
I might actually be over-estimating my high school weight out of pure defiance of how much I gained. I vaguely recall wrestling in the 130 weight class so I must have been that at some point in HS. [cries]

Do you happen to know the name for the lethal conditions that can cause? There were complications around hormone suppression in prostate and testicular cancer trials, but I don't recall any of them being lethal.
Adrenal crisis?
@Rubiksmoose ha,. although you were probably more int he 140 range if you wrestled at 130.
ANd I think I was more like 145. Because I rowed midweight.
I swam in high school and college.
I don't know the term for it, but your body needs estrogen and/or testosterone. If you have neither, that is Very Bad.
16:31
Was 145 through high school then packed on 20 pounds when I started training with my college team.
Since then Ive gained 30 pounds.
user15026
I love y'all but can we not do the weight as determiner of health sort of talk?
user15026
(this is making me, a person of size, feel super weird.)
@Rubiksmoose move messages to NAB pls?
@NautArch can do it too right?
I'm on it
Thanks mate.

@doppelgreener My recollection was the castration levels of testosterone were certainly not good for our patients. It might be that the far greater concern was the metastatic cancer so potentially moribund clinical outcomes might not have been a concern.
@Ash I'm really sorry Ash. Bad habit and something I'm dealing with right now. But no excuse for making you uncomfortable or discussing things in a way that is counterfactual. Sorry again.
@Ash My apologies as well. Intent was not to harm, and no excuse matters. I"m sorry.

Weight is a pretty good indicator for a number of health outcomes. My physician when I was playing sports that required weigh ins was particularly keen on informing me of the dangers of what I was doing.
Thinking back on it, I'm surprised she wasn't more emphatic. I think it was 35 pound delta between visits one year.
16:42
One time in college I went to see a new doctor. He was asking me about my diet and was quite taken aback when I explained that I was eating around 7500 calories a day. He started lecturing me and then I said, "I swam 35 miles last week".
Can we stop this conversation please? I moved it here, but I still think that as it has the potential to harm I'd prefer the topic of weight and health should stop. Especially if we are starting discussion about arguments people said specifically were causing harm.
It may be better to just delete them but I'd prefer not to have to do that unless it has some value.
user15026
Thank you. I know y'all likely don't mean it but seeing all y'all act like weights half my size are "bad" is not great

@Rubiksmoose I've had health problems stemming from weight and weight control. They've killed two friends of mine. I find not talking about the subject complicit in their demises. So, I'm on the other side of the fence from that stance.
user15026
Well, you're making me upset but hey you do you.
@GcL I understand your position, but let's let the topic lie ok? No good will come of arguing it here. I've had interactions with other friend groups where this topic was met with high emotions on both sides and I'd rather that not happen more than it already has here.

The assertion that it shouldn't be discussed is ... upsetting is not the right term. I feel a mix of frustration, helplessness, and maybe disappointment.
user15026
Can we...not star messages like that?
user15026
You know what? No. I'm just going to leave. I don't need this kind of stuff in my life today.
@GcL I'm truly and honestly sorry you feel that way. But the fact that you feel this strongly about it here and that other feel just as strongly in the opposite direction (and previous experience on the matter) is a very good reason not to have it continue. Nobody will be convinced and people will continue to get hurt and upset.
It's just a fact that to maintain a good chat environment, there are some topics that should just be avoided.
So, just to be super clear, this isn't up for debate. I'd like people to move on to a different topic please.
I started playing Hades a couple of days ago and that game is fun
@Rubiksmoose how about the wonderful world of having two tests in a row? High school is pain.
16:59
@doppelgreener I put it on my wishlist :)
@NautArch I bought it for three of my friends who had it on theirs!
@BardicWizard Oof. Not jealous at all of that being gone from my life.
@Ash: sorry for making you uncomfortable.
@doppelgreener ha! I'm still pretty early in playing Squadrons and Red Dead 2. ALthough this one looks like a fun family game.
@BardicWizard I was reading through Rime last night and I found some potentially triggering content. It's not as bad as what is in Strahd, but it may not be something youre okay with.
17:02
@ThomasMarkov head to the rime room...

@Rubiksmoose Throw race, gender, politics, religions, and sports teams on the list and make a channel for it?
@GcL "Not a bar in real life or on TV"
@GcL There's no need for that.
"You are specifically causing upset for someone that has asked you to stop" is a present good imperative to stop that conversation.

For now, let's just move on...to Hades.

It would be nice to have a place where those topics could come up.

They can, here and in TRPG General Chat, until it starts making someone uncomfortable or becomes outright upsetting
17:19
@doppelgreener Is there blood in Hades?
I was trying to watch the videos and didn't see any.
@NautArch Yes there is. Not during most of the gameplay, but when you die, you sink into a pool of blood, and re-emerge from a pool of blood in the halls of Hades himself.
@doppelgreener "Not during most of the gameplay, but when you die..." So this is the opposite of Dark Souls?
@doppelgreener rats. And that takes that game out of contention (my son does NOT like the sight of blood in games)
except pixellated blood, apparently that's okay.
@ThomasMarkov There's not much blood in Dark Souls either. Bloodborne or Sekiro, though....
@doppelgreener Sorry, that joke was a little layered. I meant that in your description that dying is not most of gameplay, it is the opposite of Dark Souls where dying is most of the gameplay.
17:23
Oh!
Nooooo
don't do dark souls dirty like that, lol
god, this fly. spends five minutes buzzing against the window trying to get out. i open the window and it flies away from it.
Got a doozy of a mainsite question for you guys.
"Does this SAC ruling mean that you can only make unarmed strikes with your legs?"
The ruling: "In contrast, unarmed strikes are not weapons. They are something you do with an unarmed part of your body."
@ThomasMarkov Ugh. I think i'm done with sage advice now, too.
Too many things I don't agree with. I'm just sticking by Rules as Nautarch Interprets and Make Tableplay Fun.
3
I think the melee weapon attack / attack with a melee weapon distinction was a pretty silly decision.
@NautArch Here's a clip of someone dying and coming back to the halls of Hades (35 minutes 30 seconds, if you're not taken straight there): youtube.com/watch?v=EkGMNaXg1JA&t=35m30s
@NautArch RAF: Rules As F*** sage advice
@doppelgreener Ha! ALthough I think I prefer RANIMTF.
It just rolls off the tongue.
17:53
@NautArch you're right

1 hour later…
19:07
@ThomasMarkov I see what you did there lol
@ThomasMarkov booooooooo
lol
20:07
@ThomasMarkov The latest SAC is such a disaster. (paraphrasing) You can't Divine Smite with an unarmed strike because that's not a weapon attack. Same document, a couple pages later: "An unarmed strike counts as a melee weapon attack".
WOWWWWWW
I think what actually happened is that they didn't bother reading Divine Smite before answering; it says "when you hit a creature with a melee weapon attack" but they thought it said "with a melee weapon". But they brought this on themselves by insisting that the two were different.

@MarkWells Hmm I think I made that same argument in an answer.
Ah yes I do but because of another technicality related to that

A: Does Improved Divine Smite trigger when a paladin makes an unarmed strike?

RubiksmooseImproved Divine Smite will not trigger on an unarmed strike. (luckily for the other PC) Your conclusions are correct. As the rules state, an unarmed strike is not considered a weapon of any kind. According to the PHB rules on melee attacks (post-2016 errata): Instead of using a weapon to make a ...

(the second part is about DS)
I'll have to remember to add this ruling in there. It seems silly though, they should have just errated the text.
23:27
Or, y'know, just let you go around punching people with your Fists of Justice.
"Unarmed strike paladin" does not exactly sound game-breaking.
That would be an acceptable alternative. Though it would make it differ from IDS. At my table I'd make those two consistent at least no matter if I allowed UAS or not.
SAC = Slyly Adjudicated Content
IDS = Irritable Druid Syndrome
UAS = Unproductive Alignment Squabble

user15026
See I could honestly believe those were the correct responses :p
23:42
Irritable Druid Syndrome is no joke.
the result of eating too much fruit while a bear, then turning back into a human?
Also Hades is great, the characters are all so adorably messy.
It's the second game in the roguelike genre that I've ever played more than one run of (the first, of course, is Dicey Dungeons).
user15026
@BESW that's why I love them
