I've never played Chrono Trigger, but it's weird to me that the cover art is different from the in game art because it does look to me like Toriyama did both?
like, I would assume maybe normally the art is vastly different on the cover of a game or movie or something because different people worked on it

Not.. quite.
First, about the box art.
that is the western box art. The game was released in north America about six months after the Japan release. I don't think they would make a new box art and base that on a "beta" stage. Furthermore, the few beta we know off seem to point Marle/Nadia was always an ice/life element user
The clothes, probably they simply wanted do give the characters something to wear since they put the in the middle of a snowy scene. Notice that Crono is wearing a cape in the picture which he normally doesn't have.
The enemy, probably Hekeran
Fought in a cave, present age, so that is indeed different from the picture.
As for the "fire" spell, notice that the three characters in the picture are Marle casting a spell, Frog crouching on the ground and Crono jumping.
That is actually the Triple Tech Frost Arc/Arc Impulse, not the Double Tech Frost Sword like most assume.
And for some odd reason... Frost Arc used an ORANGE/RED aura in game too.
.... or to be more precise, a flickering Orange/Blue aura

Special notices:
For some reason, Marle is casting her spell in a pose that reminds Piccolo's Makankōsappō/Special Beam Cannon in Dragonball (notice the hand pose with the two fingers). Also, in the background you can see the usual Dragonball half-spherical capsule house. There are building in Chrono with that shape, but if I remember correctly they are never seen in Chrono Trigger present era, only in the future.
Trivia: In the DS remake, a blue pallete swap of Hekeran can be actually found in a new snowy area. So, basically, you can actually replicate that scene in game in the remake.
(yep, after being so confused to call Marle Lucca more than a year ago I did some research on the picture...)
