I believe he was - for around a week. Based on Rude, pointless or lengthy chats in comments and no Comments between Nov 7 and 16. Another gap since might have been a second ban.

No. But why not just go ahead and try? You can save mid way and there is nothing to pay until all details are supplied.

I thought so too, but they all seem to be different people. At least, they've all posted different refusal notices.
