iPhone would work on the 1993 GSM network, though data would not be available (there was very slow circuit switched data available around 1995, don't know if that is supported by the iPhone, but can't think any reason to add such support even to original year 2007 iPhone), SMS might work, but probably not supported by the network. Your subscription would not be valid and 1993 SIM might not work, so you would be limited to emergency calls.

As an employer, I would not allow an employee use their own computer. I have once, and it backfired. Never again.

The one who bills is should of course be the current employer. As you are an employee of another company, it might actually be illegal to help any other company.

It is possible that the student has already cpmpleted her/his assignment and now only wants to see how professionals would do it. No, I don't even buy this myself, but you have to consider this view as well.

You say you work in a software company? Can you code? Asking because many people really can not code barely at all. Sorry to be blunt, but if implementing fizzbuzz takes more than (say) 10 mins, you should change field.

The recruiter might not edit the content of the CV, but change it to fit her company's style. I don't think anyone should have a problem with this. Also, if they reformat the doc anyway, it negates the 'word-converted version looks ugly' issue.

Medical condition goes beyond etiquette is a very good point. And not taking into account someone's medical condition is not just bad etiquette, it is simply rude.

To comment on 111111111111 being less random... it is actually possible to distinguish between real and human generated "random" strings. A real random string would contain long sequences of the same bit, whereas a human generated "random" string does not. We tend to alternate the bits more, as we think that that 111111011111 is more random than 111111111111 .
