The S'dei Chemed has a couple chapters on this issue. It was also published as a separate pamphlet called, "A Collection of Laws - S'dei Chemed, Pamphlet Metzitza and Miluim", subtitled "Which talks about the laws of Metzitza, which must be done for the Mitzvah to be performed properly, and specifically with the mouth." I haven't read it, but it looks like it addresses the question.

See this article in Yated, which claims that there is a vendetta against Metzitzah B'peh and that (at least some of) the cases of herpes related fatalities have nothing to do with Metzitzah B'peh: yated.com/content.asp?categoryid=0&contentid=686

I couldn't figure out from the articles. Was it just a scroll of Vayikra, or was it a complete torah scroll that only Vayikra remained? The it was in the inner part of the rolled scroll, but I wasn't sure what that meant. (If it was just a scroll of Vayikra, why would the beginning be in the inner part. Wouldn't it be like a megilla, where the outer part is the beginning?)

I rolled back this edit -- judaism.stackexchange.com/posts/8466/revisions -- for 2 reasons: 1) It was a quote, and editing a quote and changing it would make it not a quote anymore 2) The edit was wrong. According to the edit, there would have been no space between the plague of darkness and the death of the firstborn, and there was at least half a month. -- I wasn't sure where to put this reason for the rollback, so I'm mentioning it here

@DanF. Exactly. The wine is certified for pesach the whole year, which is why the mashgiach is vigilant. The butchery is not, and therefore the mashgiach is not.

when something is not forbidden you aren't careful about it. In the middle of Tishre the worker doesn't think twice about bringing chametz in. If you want to ensure that the food has no chametz, clean it for pesach before you start selling it for pesach.

@HodofHod - I just had a crazy thought, which might be out of scope for the Referencer. I have a little script to automatically generate a hebrewbooks URL from the book ID and the pdf page number:
