what would be a system to handle duplicate questions?
I think some way to list 'similar questions' similar to SO etc. would be useful but not sure how to implement one
perhaps some sort of unsupervised text clustering ML algorithm?

I don't think that's something we need to worry about right now, but it's good to think about. Text clustering is probably the best bet.
