Is the Swedish warship Vasa used as an example outside the Nordics? It sank early into its maiden voyage, due to poor design which was influenced by rushed schedule and constantly changing specifications. Sound familiar to system engineer people?

There was a pizza order site that got into a bit of trouble back in 2010 and around, when it turned out their ordering UI allowed giving negative tip which'd actually be deduced from the order's total cost

I was a bit amused to find that out because it reminded me of the money lender bug from the original Tai-pan! where the game permitted repaying the money lender in excess to amount actually owed, resulting in a very-high interested investment :)
