This is a very common problem and the sad truth is that most companies will avoid promoting you if possible because it doesn't benefit them to do so. It likely means they have to pay you more and it makes you more attractive to other companies if you plan on leaving. It's important to understand this and that promotions based on your work ethic alone are very rare.
