Actually it went okay
I ended up using a cleat bolt and grinding it down - the sides are ~1mm thick so it was a fiddly job
I drilled clearance for a M4 bolt down inside the 6mm hex, and gobbed the hole with 2 part epoxy. I didn;t think threadlock would work here.
Part-way through grinding:
yes I used an old er16 collet chuck to hold the M4 thread.
I've tried the chaintool in the garage, and it worked fine. Have to try it for real before I trust it on the commute though.

....Semi-related question - Does there exist a ER40 or ER32 collet holder with a Morse taper 1 on the back?
Most I can find is a single ER25 on a MT1 but it lacks a drawbar thread. Everything else stops at ER20.

I have found exactly one - Mt1 to ER32 collet set - but its 130 UKP. So they do exist.
