I too find it really odd that the coworker or boss could not tell about this client. Or that the client would not host "safe for work" test content, too.

It is baffling why neither your co-worker at the centre of the question, nor any of your other colleagues, nor your boss, nor HR, were able to explain about the client which was apparently common knowledge to all of them - and why your VP implies you did something wrong by not knowing about it, when none of them told you (some document somewhere mentioning it, or not). FWIW, I don't think you need to feel stupid - I think by going to HR you did the right thing with the information available to you at that time. I think everyone else in your company should feel stupid though.
