Even though I wasn't asked I'm gonna tell you. I think you should provide the source of that entire thing.
If something (doesn't matter what and how small it is) was not created by you then you have to provide proper attribution.
Otherwise it is plagiarism.
Which is a big no-go.
I think it would even be in violation with the CC license.
