My idea about Cleanup Crew had been wrong like the edited version of my answer. I have deleted yesterday and today together 42 comments on SO which had been from 2013 and got the hat.
So my guess: it had to be 40 comments older than one year and needs to be deleted in two days in a row. @Pops

@pops - If an hat required a "network wide" action, it would show as awarded on meta? Suppose an hat was given for joining X sites, you would earn it on meta I suppose, right?
To put it in another way: an hat that is displayed as awarded on meta doesn't actually have to be triggered by an action you did on meta. Meta could just be a placeholder for something network wide, right?

Basically what it is: a series of text files, each named by their timestamp, with a list of secret hats, the user who earned that hat, and what page of the leaderboard that user was on.
