Very likely doable from the SmokeDetector side of things
but to actually make it do something on the MS side is nontrivial and painful most likely
so that needs @Undo and @ArtOfCode's opinions specifically, less so than mine
because the MS interaction bits are its own can of worms / circle of (coding) hell.
@gparyani also, you'd break the existing function
 
But if you exclude the part to add in the manual reason to metasmoke, would it be easy to only propagate the reason field in Smokey's chat messages?

No. Most mods probably never notice that there's been spam deleted on their site unless it gets mod flagged in a way that persists after the post is deleted by Charcoal people.

Why bother specifically with this one? Out of my 8000+ spam flags on Drupal, I think 7500+ still have an existing account.

I'm not sure what the added value of that is
They post spam, it gets removed, regardless of whether it is a new account or not
making a new account is fairly trivial
and if they post spam on the same account twice, the username was already blacklisted, which makes it easier to detect and autoflag
