@Shree The reason was "Flag option not present" (you can see that by hovering the mouse over the "Fail"). That response is because the post was deleted prior to the system attempting to flag using your account. This page shows that MS raised 3 flags on that post, then went through the rest of the users who had flagging conditions that matched, trying to put the fourth flag on the then-deleted post.
Unfortunately, MS does not try to detect when a post is deleted prior to raising each of the flags it believes it should. Thus, it just keeps trying with every account that has an autoflagging condition that matches the post.
